• what sort of support you may need and what’s available near you
• about paying for care
• what happens when you leave hospital
• how to buy equipment to help you around your home
• what type of support care homes can offer

The assessment will give us information about your situation and what changes you would like to make in your life. To refer either complete a self referral (can be friends or family that refer too) or ask a professional to help.
